Every time I rise the fame (i'm guessing that is what happens) when I talk to the female NPC right besides the clown sometimes she happens to have a quest on her and I will be rewarded with a item which I'm still unable to understand what is it for.

Use it when you kill the last BOSS in the daily "DUN" (Gold Zodiac, 10th Wind Cave, Saggitarius Heart), you'll spawn another BOSS who gives better things.
